The Transplant Nurse Coordinator coordinates care of solid organ (heart, lung, heart-lung, liver, kidney, pancreas, and multi-organ) transplant patients, both adult and pediatrics, through out the phases of the transplant continuum.
The role may include facilitating referrals for evaluation, assuring that adult and pediatric transplant patients needs are met during the inpatient stay, and for the long term follow-up after discharge from the transplant admission. In this role s/he will be responsible for educating patients, family and other providers about the intricacies of organ transplantation. The ability to communicate to a diverse audience, synthesize and apply advanced clinical knowledge, be flexible, and collaborate in an organized manner is essential to the role. Responsibilities also include on-call coverage to meet patient needs beyond the daily business hours. As a member of the multidisciplinary transplant team, the Transplant Nurse Coordinator plays an integral role in achieving safe, high quality patient outcomes through teaching, research, and leadership. Education BSN required; Masters Degree in Nursing preferred.Experience5 years nursing experience with at least two years in transplant, critical care, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific. Case management or clinical research experiences also a positive.
