Advocate Lutheran General Hospital is currently seeking candidates for a Clinical Pharmacist position.
The Infectious Diseases (ID) Clinical Pharmacist will be accountable for working with pharmacy, physicians, nursing, and senior leadership to develop and maintain a patient focused multimodal service, with a focus on management of medical and surgical patients. The ID Clinical Pharmacist ensures appropriate use of antimicrobials in the hospital, both globally and on a patient-specific basis. The ID Pharmacist works closely with Division of Infectious Diseases physicians to formulate guidelines/procedures to ensure appropriate use of antimicrobials throughout the hospital (e.g. empiric therapy guidelines, surgical prophylaxis guidelines, etc.). Other responsibilities include monitoring antimicrobial therapies of patients with positive blood cultures or multi-drug-resistant organisms, and monitoring use of specific antimicrobial therapies (broad-spectrum or high cost agents) for appropriateness. Additional activities include: education of healthcare professionals including pharmacy residents and students, attending and house staff physicians, and nurses; member of several hospital and health-system-wide committees; involvement in Pharmacy and Therapeutics Committee regarding antibiotic formulary issues.Qualifications: - General knowledge of computer applications- Experience with presentations- Understanding of a hospital environment - Doctor of Pharmacy degree (Pharm.D.) or Masters in Science- State registration as a pharmacist obtained within 60 days of hiring- ASHP accredited pharmacy practice residency - Advanced residency training or fellowship in infectious diseases is preferred.Advocate Lutheran General Hospital, recipient of the prestigious Magnet designation, is a 617-bed facility and Level I Trauma Center.
