A.DESCRIPTION/SPECIFICATIONS/WORK STATEMENTa. Performs as an Inpatient Pharmacist with full professional authority, knowledge and judgment, with advanced level of responsibility and experience in all aspects of the total Pharmacy operation. Serves as the pharmaceutical expert in such areas as medicine, surgery and geriatrics including the recommending, calculating and preparation of TPN and PPN nutrition support products, chart reviews and monitoring and assessing the outcome of drug therapies.b. Incumbent may perform inpatient departments, IV admixtures, unit dose and automated drug distribution systems, procurement, consultation, stock control, research, bulk compounding, prepackaging and storage.c. The incumbent is assigned special reports, projects and problem matters, requiring an overall or specialized knowledge of the trends, problems, and other matters affecting the total Pharmacy operation.d. Responsible for the receiving storing, compounding, dispensing and accounting of all medications, narcotics, investigational drugs, and supplies.e. Provides clinical information on a timely basis to all hospital staff and patients.f. Inspects wards and clinics for outdated, deteriorated or excessive stocks.g. Handles problems in a discreet manner and ensures a satisfactory solution. Refers problems above his/her authority to supervisors.h. Performs special assignments when assigned, such as drug utilization evaluations, systematic internal reviews and AMIS reporting.i. Demonstrated knowledge, skills and abilities in the following is necessary:(1)Ability to read, interpret and apply complex written instructions(2)Knowledge of professional pharmacy practice(3)Ability to communicate orally and in writing(4)Basic knowledge of pharmaceuticals, pharmacokinetics, pharmacodynamics and pharmacotherapeutics; (5)Basic computer skills;(6)Ability to analyze drug-related medical problems.(7)Skill in preparing sterile products(8)Knowledge of standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs.) This includes, but is not limited to, those established by the Drug Enforcement Administration (DEA), Food and Drug Administration (FDA), Department of Veterans Affairs (VA), the State (Territory or District of Columbia, if appropriate), and the Joint Commission (JC).CLINICAL FUNCTIONSa. Evaluates medical orders for appropriateness of therapy, incompatibilities and drug interactions. Takes corrective action in all obvious instances of inappropriate therapy, incompatibilities or drug interactions.b. Provides accurate, unbiased and cost-effective drug information to health care providers and patients. Provides patient consultations to improve patient understanding and compliance in their drug regimen.c. Provides specialized clinical support to physicians, which includes, but is not limited to, pharmacokinetics projects, chart reviews, nutrition support consultations, and ambulatory patient consults. Performs 30-day chart reviews for NHCU residents and 24-hour profile check and polypharmacy reviews for acute care wards. Advises providers of appropriate medication therapy changes, monitors and assesses the outcome of drug therapies including physical assessments and interpretations of laboratory and other diagnostic parameters.d. Interprets, evaluates, and applies an extensive spectrum of professional knowledge to the processing and manufacturing of a broad array of parenteral solutions which include large volume parenterals, piggyback parenterals, hyperalimentation solutions, chemotherapy solutions, syringes, renal dialysis solutions and irrigation solutions. Provides accurate and timely IV admixture information to all health care system staff.DISPENSING FUNCTIONSa. Evaluates physicians´ orders as to appropriateness based on patient medication profiles, inputs information into computer system if assigned, dispenses medication in accordance with dosage form requirements.b. Provides quality control analysis for dispensing activities of automated dispensing equipment of pharmacy technicians.c. Follows all aseptic techniques when admixing sterile solutions and chemotherapy.d. Receives and accurately interprets all written orders received from physicians, dentists, and other health care professionals utilizing professional knowledge of drugs and VA policy and procedures contacting the practitioner when an order is unclear or questionable.ADMINISTRATIVE FUNCTIONSa. Resolves problems in day-to-day routines associated with the drug distribution process such as unusual usage patterns, inadequate dosage forms, missing doses or other general operational problems. Monitors and reports utilization of pharmaceutical products to maintain adequate supplies. Submits solicited reports on time.b. Provides strict security of all controlled substances by reviewing orders and records. Verifies daily that perpetual inventory of all Schedule II through Schedule V narcotics is accurate. Uses prescribed record keeping, controls, and knowledge of rule and regulations regarding controlled substances.c. Maintain accurate records of all workload accomplished in the IV area. Reports information in appropriate AMIS format when required. Ensures IV and chemotherapy solutions are manufactured in accordance with the latest acceptable standards and are delivered on a timely basis to appropriate personnel.d. Directs and inspects the work of technicians involved in the technical functions of Pharmacy to ensure accurate and timely pharmaceutical services.e. Guides and directs, orients and trains employees, students and residents to ensure acceptable quality and quantity of work and in keeping with professional and VA standards. Ensures that all areas of responsibility are functioning properly and training conducted efficiently.
