Works under the direction of the Pharmacy Clinical Coordinator. Provides pharmaceutical care to the veterans in the ambulatory clinic setting. The incumbent will promote, assess and assure quality and cost-effectiveness of drug utilization as well as ensures that drug selection is in compliance with national Formulary, VISN 18 directives, Quality Improvement, JCAHO directives, OIG requirements and clinic procedures. Performs pharmacy clinical duties which may include providing clinical services for the Healthcare System within area of clinical practice expertise. Provide patient consultations, evaluating medication regimen for appropriateness of use, drug interactions, therapeutic duplications, non-compliance to drug therapy and adverse drug events. Haves knowledge of the standards related to distribution/control of scheduled and non-scheduled drugs as well as all standards that are established by Federal, State, VHA, DEA, FDA and JCAHO. A COMPLETE FUNCTIONAL STATEMENT IS AVAILABLE FOR REVIEW IN HUMAN RESOURCE MANAGEMENT SERVICE.NOTE: Five positions are located in Ambulatory Care. The sixth position is located in Mental Health. The psychiatry clinical pharmacist serves as a consultant to physicians and other health care providers in Mental Health Service. S/he will provide direct and indirect patient care activities, assessing the efficacy, safety and appropriateness of medication therapy.
