North Texas Health Care System (VANTHCS), Dallas VA Medical Center is seeking to recruit a full time Pharmacist to work night shift as an Inpatient Pharmacist. Incumbent must have the ability to read, interpret, and apply complex written instructions; knowledgeable of pharmaceuticals, pharmacokinetics, pharmacotherapeutics; ability to analyze drug-related medical problems; knowledge of standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs). Incumbent must have an extensive knowledge of inpatient hospital experience in pharmacy. Responsible for patient profile review, obtaining and documenting allergy and adverse drug reaction information in the current computerized patient record. Will organize workload to assure optimum, accurate and efficient completion of all medication orders, both inpatient and discharge. Will provide quality control checks and analysis of automated dispensing equipment as assigned; fill and issue ward or clinic orders for drugs, supplies, and narcotics as necessary, and will maintain records accurately. He/She will also serve as a liaison between the inpatient areas, primary care clinics, or other patient care areas and the centralized pharmacy distribution area, identifying and correcting problems and improving communication. Flexible Schedule: -8 days on & 6 days off, 10 hour shifts, Monday through Monday-7 days on & 7 days off, 10 hour shifts, Monday through Sunday or -Tuesday through MondayYou must be a citizen of the United States. In addition, applicants must all meet all of the requirements for the position as stated in VA Handbook 5005, Part II, Appendix G15 and have full, current and unrestricted license to practice pharmacy in a state, Territory, Commonwealth of the United States or the District of Columbia. Minimal qualifications requirements include the completion of the equivalent of one (1) year of professional pharmacist experience at the next lower grade level that has provided a broad knowledge of the complete pharmacy operation. Experience may have been gained as a clinical pharmacist in a major specialty area or as a pharmacy specialist responsible for a major program area within a pharmacy service. Qualifying experience may also have been obtained as a pharmacy supervisor responsible for the professional and administrative management of an inpatient/outpatient area in a large pharmacy service or an inpatient/outpatient area at a site geographically removed from the supervisor.

Works under the direction of the Pharmacy Clinical Coordinator. Provides pharmaceutical care to the veterans in the ambulatory clinic setting. The incumbent will promote, assess and assure quality and cost-effectiveness of drug utilization as well as ensures that drug selection is in compliance with national Formulary, VISN 18 directives, Quality Improvement, JCAHO directives, OIG requirements and clinic procedures. The incumbent will be responsible for providing pharmaceutical activities and services in clinic settings. These activities and services include the provision of clinical pharmacy consultations on pharmaceutical usage, disease states and drug preparations for patients. The incumbent is also responsible for the daily operational functions of his/her assigned program and will continually monitor and assess the overall operational effectiveness in terms of quality, quantity, timeliness and cost containment. Duties include but are not limited to: The incumbent is responsible for planning, implementing, and operating the drug monitoring and utilization programs on the inpatient floors. The incumbent is the expert in all areas related to drug therapy and functions as a consultant to other health care professionals. The incumbent will also be responsible for developing, providing, and supporting the educational programs of the Pharmacy Service. Responsible for medication dispensing and distribution. Participates in daily multidisciplinary team rounds (when applicable) and provides drug information, observations on patient response to therapy, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomics. Provides consultative services to all professional staff regarding drug therapy. Serves as a preceptor when appropriate for the students and residents at the Marion VA Medical Center. Participates in the development and implementation of formal and informal drug education/clinical consultation to medical, surgical, nursing, and pharmacy staff in the form of in-services and continuing education programs. Manages the hospital formulary and ensures its appropriate utilization by health care providers. Documents cost reduction/cost avoidance activities on a periodic basis.

The VA Medical Center is located on a 173-acre campus in West-Central Wisconsin, Tomah VAMC is a 271-bed facility focusing on primary care, mental health services and nursing home care.
