POSITION QUALIFICATIONSMinimum Education/Qualifications Graduation from an accredited college of pharmacy Licensed to practice pharmacy in the state of Kansas Numerical abilities, form perception, and basic knowledge of computers and word processing skills Possess leadership, interpersonal, and organizational skills Able to interpret written and oral prescriptions Able to work in a fast paced and an interruptive environment while maintaining focus and accurate work Previous hospital experience preferred Minimum of 30 days is required to familiarize the candidate with all administrative responsibilities and functions of the pharmacy Completion of required hours of CE for re-licensure Maintains personal professional liability insurance Annually completes competencies as required by the job description CORE RESPONSIBILITIES Prepares and dispenses medication orders per physician request according to established policies, procedure, and protocols. Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Reviews appropriateness of drug, dose, use and other information when interpreting medication orders Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately. Monitors medication use and initiates interventions when needed Monitors dosing with regard to disease state, patient age/developmental stages of the neonate, infant, child, adolescent, adult, and geriatric patients Monitors drug therapy regimens for contraindications, drug-drug,drug-food, and drug-laboratory test interactions,patient allergies, appropriateness of drug and dose, therapeutic duplications, and performs appropriate clinical follow through Reviews/interprets culture and sensitivity data for antibiotic appropriateness and recommends changes as needed. Reads, extracts, and interprets information in the patient chart accurately. Detects and reports suspected ADRS and medication errors, including POTENTIAL errors (near misses) in a timely manner. Sustains the formulary by minimizing nonformulary procurements and utilizing therapeutic substitution protocols per P&T Committee. Demonstrates good oral and written communication and drug counseling skills, including documentation of communication Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff, as needed. Provides accurate, adequate, and timely drug information to the professional staff. Provides drug information to patients and their families. Monitors pricing and takes appropriate action when needed. Maintains records of narcotics and other medications as required by law Compliance with the hospital policies for safe preparation and handling of NIOSH drugs, chemotherapy, alcohol,investigational drugs, home meds and non-formulary drugs. Participates in the performance improvement and DUE activities of the department. Collects data, interprets and reports results. Offers suggestions, initiates, and /or develops changes to improve processes within the department or hospital Supervises, directs, and verifies the daily activities of pharmacy technicians. Assists in the orientation, education and training of new Pharmacy employees, SOP students, SON students. Participates in the education and training of new nursing staff. Participates in planning and goal setting for the department. Demonstrates a high level of productivity and makes good use of work time Limits personal business, phone calls and Internet use while on hospital time.
