Position Summary Overview: Promote pharmaceutical care by managing the medication process, identifying and preventing potential drug-related problems and resolving actual drug-related problems. Responsibilities: Ensure the appropriateness of medication doses in reference to the patient´s diagnosis, weight/body surface area, allergies and drug interactions. Enter medication orders, ensure orders are processed and medications received by the patient are correct. Follow aseptic technique and infection control in the preparation of sterile products. Participate in the education of patients, care givers and the community by providing drug information. Participate in the training of new employees, education of pharmacy students and the education of other healthcare professionals. Publish one article in a professional journal/newsletter or makes one presentation at a community or professional meeting annually. Support patient safety systems by completing intervention documentation. Initiate and processes medication incident and adverse drug reaction reports. Perform and document patient profile review. Complete pharmacist code response intervention after responding. Perform and document discharge prescription review. Complete VI (Value Initiative) data collection form. Enforce the non-formulary medication guidelines. Act as a liaison between the pharmacy, the nurses and physicians. Engage in shift management by ensuring the pharmacy runs smoothly and by maintaining adequate stock of medications and supplies. Follow established procedures for stock replacement and non-formulary orders. Exhibit leadership in coordinating on-line activities including technician workload, managerial duties and shift transition. Re-establish priorities based on workload and patients´ needs. Maintain investigational drug records. Knowledge and Skills: Medication knowledge base, knowledge of pediatrics and proper handling of hazardous substances. Maintaining validation of departmental and area specific competencies. Incumbent must be able to work under pressure and work closely with others continuously throughout the day. Education, License and Experience: Degree in Pharmacy; licensed to practice pharmacy in Texas is required. Hospital experience strongly preferred.
