Provides consultation in palliative care, symptom and supportive care to meet the needs of the patient as requested by the referring physician. Facilitates clarification of patient and family goals of care and the access to appropriate support models of care in hospice, rehabilitation´s, and on-going aggressive treatment. Educates facility staff, co-workers and the community on all aspects of palliative care and the services provided by Vitas Solutions, Inc. Prescribes medications and treatments according to patients palliative care needs and in collaboration with the referring physician. Orders diagnostics/treatments as needed after collaboration with the referring physician and assists in development of clinical practice guidelines, standards in support of quality palliative care as requested by the supervisor. Prepares and maintains accurate patient records, charts and documents to support the medical practice and reimbursement for services provided. Adheres to the practice of confidentiality regarding patients, families, staff and the organization and participates on-call rotation as needed. Supervises palliative care nurse practitioners and physician assistants.QUALIFICATIONS: Doctor of Medicine or Osteopathy with two years experience, licensed to practice medicine in the state. Advanced and ongoing training in palliative medicine, diagnostics skills, symptom management and communication skills. Knowledge of the interdisciplinary team concept.
