The group provides inpatient management for acutely and critically ill patients . The group provides valley wide coverage on a 24 hour, seven (7) day per week basis to twelve (12) acute care hospitals and three (3) sub-acute/skilled nursing facilities.The Hospitalist manages each case utilizing other specialists as needed. Hospitalist physicians have no private outpatient clinical practice. In addition, the Hospitalist is partnered with an RN Care Partner who assists in facilitating both inpatient and outpatient tests and procedures. Upon discharge, the RN coordinates any necessary appointments and provides a discharge follow-up phone call to the patient to ensure that all discharge instructions such as prescriptions, durable medical equipment (DME) or home health care (HHC) are received and followed as directed by the physician. Patient load varies depending on shift; average range of 15-20 patients/day/physician. Unlike assembly line practices, physicians are encouraged to spend reasonable time with patients.Teams of physicians service the facilities. Team sizes are determined by the volume of patients at the facilities. Physician teams, led by a Team Leader, determine among themselves the number and types of shifts individual team members will work to ensure continuity of patient coverage 24/7 for their assigned team.Quality and performance incentive pay is based on the individual´s performance in conjunction with the overall performance of the organization. Essential Duties and Responsibilities:1.Hospitalist in the hospitals Round in the hospital from 8am to noon, and sees an average of 6 to 10 patients. Conference calls with Case Managers to review patients, and discuss the discharge needs and plans. Admit the patients from the ER patients in the afternoon (usually 2 to 4 patients), if they are assigned float position for the given day. Work with Case Managers in transferring the patients from out of area hospitals into network hospitals. 2.Hospitalist in the clinics See patients in the CareMore Care Center (CCC.) All patients discharged from the hospital are seen by the hospitalists in the clinic until they are stabilized. Patients with falls are assessed. Pre operative clearance is done on patients undergoing surgeries requiring general and spinal anesthesia. Assist Nurse Practitioners by reviewing the cases with them. 3.Hospitalist in the Skilled Nursing Facilities (SNFs) See the skilled patients in the SNFs. These patients are seen once a week until they remain skilled, which is normally from 1 to 2 weeks. Attend the SNF meetings once a week to review the cases. 4.Calls Night calls for 1 week approximately every 3 months. No day time duties during night call week. Weekend calls approximately 1 in every 3 to 4 weekends. This involves rounding at 1 or 2 hospitals on Saturdays and Sundays and taking calls until 5 pm.
